Optimizing Care Management

Introduction:

Caring for patients with complex medical needs requires additional supports. There are a suite of tools available to teams to help them optimize their care management and prescribing. Tools may be useful prior to initiating opioids, while maintaining opioids, and in the care planning process.

Learning Objectives:

  1. Describe the benefits of care planning and how it can be incorporated into a practice
  2. Apply PBPs that support safer prescribing practices

Harm Reduction Module

The Harm Reduction Module for Primary Care was created by Alberta Health Services. It includes evidence based strategies for supporting a harm reduction approach within primary care settings, including recommended practices for reducing the experience of bias and stigma faced by People Who Use Drugs, their families and other individuals with lived experience of opioid use.

The module is intended for a broad multi-disciplinary audience including physicians, nurse practitioners and other health care providers. It will build on foundational principles of harm reduction and work already accomplished through the process of revising the AHS Harm Reduction Policy.
