Practice Facilitation Resources

Overview

Practice Facilitators are specially trained individuals who work with community care practices to enable meaningful changes designed to improve patients' outcomes and clinic processes. They help physicians and quality improvement teams develop the skills they need to implement and adapt evidence-based practices to the specific circumstance of their practice environment. They are a key resource in the transformation efforts toward the Patient's Medical Home and Health Neighbourhood.

ACTT is pleased to provide a suite of practice facilitation tools and resources for practice facilitators and the organizations that support them.

Practice Facilitator Network

Alberta Practice Facilitators are partnered with ACTT (Accelerating Change Transformation Team) to deliver a provincial Practice Facilitator Network.  It is a venue for Practice Facilitators to connect, share learnings, and collaborate.

The PF Network launched October 2017 and is predominately virtual in nature, including monthly interactive webinars and a collaborative website.  The success of the PF Network is grounded in participants' commitment to engage and communicate their collective experience, wisdom, and passion to enable meaningful changes designed to improve patient outcomes and clinical processes in primary care for the transformation toward the Patient’s Medical Home integrated in a Health Neighborhood.

The monthly webinars cover topics related to practice facilitator competencies and emerging provincial priorities, such as provincial continuity data, quality improvement dashboards, HQCA panel reports, CPAR implementation, Patients Medical Home assessments, and data storytelling.

Practice Facilitator Connect

Practice Facilitator Connect is a regular meeting for anyone needing support with their role as a Practice Facilitator. This meeting is hosted by ACTT and aims to support Practice Facilitators by creating a forum to connect and ask questions. The sessions also offer tips, tricks, best practice, and engagement with ACTT consultants and colleagues across the province. Those that are taking the Practice Facilitator Core Training: Comprehensive can use this time to ask questions about materials covered during the courses.

These meetings typically run bi-weekly (except for November and December 2023) on Thursdays at 1:00 p.m. You can choose to attend one meeting or all of them. See the dates and register now
