Overview

The Patient’s Medical Home implementation element of ‘Organized Evidence Based Care’ involves embedding evidence-based guidelines into daily clinical practice where each encounter is designed to meet the patient’s preventive and chronic illness needs.

The following elements are key to practicing 'Organized Evidence Based Care':

• Identifying high risk patients and ensuring they are receiving appropriate care case management services
• Using point-of-care reminders based on clinical guidelines
• Enabling planned interactions with patients by making up-to-date information available to providers and the care team at the time of the visit
