Practice Facilitator Core Training

ACTT offers two options for this training:

• Comprehensive
  • For those working directly with clinic teams to facilitate quality improvement.
• Self-Directed
  • For those who do not directly facilitate quality improvement with clinic teams or those that have taken this training before.

Why is Practice Facilitator training important?

• Practice Facilitators play an essential role in supporting clinic teams in their Patient's Medical Home transformation journeys.
• This training introduces the core competencies Practice Facilitators need to support clinic teams including:
  • Context of primary care
  • Patient’s Medical Home and Health Neighbourhood
  • Knowledge and practical skills in quality improvement and modes of influence.
