Accelerating Change Transformation Team (ACTT)

Who We Are

We are a dynamic change management team committed to health system transformation for the benefit of all Albertans. 

The Accelerating Change Transformation Team (ACTT) is funded by Alberta Health. 

What We Do

We work with physician leaders, clinic teams, Primary Care Networks (PCNs), and health system partners across the province to simplify, support, and scale provincial health care transformation projects and innovation. 

We provide the following change management and transformation supports:

Strategic Supports & Physician Leadership

  • Provide strategic supports to physician leaders at multiple levels of Alberta's health care system - provincial, zonal, PCN, and clinic levels.
  • Partner at provincial and zonal tables, providing strategic advice to align approaches that lead to health care system transformation.
  • Provide strategic transformation supports to various zone and PCN boards and committees.
    • This involves:
      • seeking to understand zone needs;
      • demonstrating how priority health care initiatives connect to those needs; and
      • aligning change management activities to zone service plans.
  • Develop and share guidance and resources on change management best practices in the clinical context with Primary Care Networks and clinic teams.
  • We simplify and translate complex system change initiatives to practical steps.

Training, Tools, & Networks

  • Offer online and in-person training, tailored to the Alberta context, to support physicians, PCNs, healthcare professionals and clinic staff with health system transformation in Alberta.
    • Topics include:

      • CII/CPAR
      • Care Coordination
      • Privacy
      • Practice Facilitator (PF) Training
      • PCN Governance
      • And more!
  • Create and share tools and resources that provide practical guidance for effective change management and implementation in the clinical context.
  • We develop and foster networks to build capacity in the health transformation workforce, and to facilitate peer-to-peer conversations and learning.

ACTT's Current Projects

ACTT's 2023/24 projects are:

Community Information Integration & Central Patient Attachment Registry (CII/CPAR)

We help physicians (including non-family medicine specialists) and clinics adopt the technology that enables them to share patient information, participate in enhanced communication among providers, and optimize continuity of care across Alberta's health care system. Learn more.

Care Coordination

We provide advice, supports, and tools for physicians and health care teams to enhance care coordination, including home-to-hospital-to-home transitions and access to specialty care. We promote the importance of building effective and efficient connections between the Patient's Medical Home and the Health Neighborhood. 

Vital Clinics

We facilitate a peer-to-peer learning program aimed at sustaining and strengthening the practice and application of the Patient's Medical Home in clinics, with a particular focus on improving access and leveraging a team. Learn more. 

Blended Capitation Model

We support change management activities for Patient's Medical Homes interested in an alternative to the fee-for-service model, enabling more flexibility to be innovative in how service is delivered to patients. Learn more. 

Our History

ACTT (Accelerating Change Transformation Team) was established on November 1, 2018.

ACTT is a change management program within the Alberta Medical Association (AMA), connected to AMA's key focus on health care system leadership and partnership. ACTT is the result of a merger of three previous support programs - Practice Management Program, Toward Optimized Practice, and PCN Program Management Office. ACTT was created to better integrate the support and change management services of those former programs.

In its various forms, ACTT has supported practice management, clinical quality improvement, and health care transformation in Alberta for over 20 years. 

ACTT is funded by Alberta Health.
