Learning objectives

By the end of this curriculum, learners will be able to:

  • Explain the concepts of and evidence for the Patient's Medical Home and Health Neighbourhood. 
  • Describe the role primary care and Primary Care Networks play in the healthcare system and its transformation.  
  • Explain how and why change agents need to work together to achieve system transformation.
