In the Patient's Medical Home approach, care coordination is achieved through a set of activities that minimizes the risk of fragmentation by ensuring that all providers involved in a patient's care share important clinical information and have clear, shared expectations about their roles. To coordinate care, a PMH practice acts as the 'hub' of care, and develops relationships within the Health Neighbourhood by using co-developied protocols to achieve seamless referrals and transitions.
