Learning Objectives

By the end of the Practice Facilitator Core Training: Comprehensive, you will be able to:

  • Explain the context of primary care in Alberta, including clinics, Primary Care Networks and provincial structures.
  • Describe essential concepts and evidence for health system transformation including Patient’s Medical Home, Health Neighbourhood, Change Agents, Panel, Continuity and ACTT Change Packages.
  • Implement the ACTT Sequence to Achieve Change with clinic teams to support their Patient’s Medical Home process improvements and to build their internal capacity for continuous quality improvement.
  • Utilize data, measurement and quality improvement tools to help clinic teams to sustain, spread and scale successful process improvements.
  • Apply change management principles and approaches to develop strategies for engagement that meet clinic teams where they are at.
  • Apply coaching and facilitation tools and techniques to contribute structure and process to team interactions, allowing them to function effectively and meet their goals.

45 hours.

Important Pre-registration Details

To receive a certificate of achievement, all learners must:

  • Complete all Learn@AMA courses prior to corresponding interactive sessions.
  • Attend all interactive sessions (virtual and in-person).
  • Complete Practice Facilitator Self-Assessments.

Please note that the certificate of achievement is given after the virtual celebration and completion of the Practice Facilitator Self-Assessment.
