Practice Facilitator Core Training
Practice Facilitators (PFs) play a key complementary role in supporting clinic teams in their transformations to Patient's Medical Homes (PMHs). These courses introduce the core competencies PFs need to support clinic teams – i.e., the context of primary care, the Patient’s Medical Home and Health Neighbourhood (HN) and knowledge and practical skills in quality improvement and modes of influence.
