PCN Strategic Forum 2021

The PCN Strategic Forum is an annual two-day conference that focuses on the PCN and Zone contributions to the transformation of primary and community health care in Alberta.  PCN, zone and system leaders gather to learn about the important initiatives ahead and better understand the work already underway. Whether at the system level or at the clinic level, primary care plays a vital role in the healthcare of today!

In May of 2021 the Forum planning committee made the decision to gather virtually because of pandemic related travel restrictions.

Virtual Care

Virtual care has grown since the onset of the pandemic and is another enabler of continuity of care and many of the Patient's Medical Home goals we seek to achieve.
Information on
virtual care including webinars, EMR details, a toolkit and more.

Vaccine in clinics was piloted in later April and shown to be very successful.  The vaccine will be rolling out to more clinics very soon.

Alberta Health Services has been the primary source for COVID-19 information and hosts clinical resources for all providers, including those in primary care.

AMA website hosts a resource centre with webinars and other tools including the "Be a Vaccine Positive Clinic" Toolkit to help patients overcome vaccine hesitancy.

         Home to Hospital to Home (H2H2H)

The H2H2H Transition Guideline was approved by Provincial PCN Committee and AHS Clinical Operations Executive Committee earlier in 2020.

 Alberta Surgical initiative (ASI)

This is a very large project being led by AHS and Alberta Health and is in the development phase.

 Rural Sustaibability Task Force

The RSTF is a multi-stakeholder group constituted by the Provincial PCN Committee to provide recommendations on topics relating to the sustainability of primary care delivery in rural Alberta, to the Minister of Health by 2022.
