A Practice Facilitator (PF) plays a key complementary role in supporting clinic teams in their transformation to Patient’s Medical Homes. PFs guide and support member clinics as they take incremental steps toward sustainable process improvement. PF training provides participants with practical skills and knowledge, including: the Patient’s Medical Home (PMH) approach, quality improvement (QI) tools, facilitation skills, administering the PMH assessment, and much more. PFs are also trained in facilitating ‘change packages’, such as Opioid Process Improvement, Patient’s Collaborating with Teams (Care Planning), Continuity, Alberta Screening and Prevention (ASaP & ASaP+), and more.
