Sequence to Achieve Change

Introduction:

Achieving change in a clinical setting is difficult as it requires changes in behaviours and routines. Practice Facilitators can use the Sequence to Achieve Change and quality improvement skills and tools to support teams in this effort. 

Learning Objectives:

  1. Demonstrate how to apply the Sequence to Achieve Change.
  2. Work with a team to implement a meaningful measurement strategy for quality improvement work.
  3. Plan and execute plan-do-study-act cycles (PDSAs).
