Practice Facilitator Network

​Overview

Accelerating Change Transformation Team (ACTT) is  partnering with PFs to develop a provincial Practice Facilitator Network for Practice Facilitators to connect and allow shared learning, collaboration and rapid spread of promising ideas.

The PF Network launched October 2017 and will be predominately virtual in nature, in the form of short, monthly interactive webinars.  The success of the PF Network will be grounded in participants' commitment to engage and communicate their collective experience, wisdom and passion to enhance the care of patients in primary care.

These monthly webinars will cover topics such as provincial continuity data, quality improvement dashboards, HQCA panel reports, CPAR implementation, Patients Medical Home assessments, data storytelling.

Join the PF Network website on the PCN PMO website to access current and future PF Network materials.

To gain access to the IF Network page:

  1. Navigate to the Membership Request page on pcnpmo.ca
  2. Fill in the requested information and click the “Request Account” button.
  3. Once approved, this account will grant you access to secure pages of www.pcnpmo.ca and/or a Primary Care Network website
  4. Once you have your login information you will then go to the webpage at https://pcnpmo.ca and click on the tab labelled Collaborate then click on IF Network.

To set up a notifications to the discussion board.

  1. Navigate to https://pcnpmo.ca. Login using the link in the top right hand corner, and select IF Network from the Collaborate tab.
  2. On the discussion board page, click the link to Create a notification.
  3. Make sure the Change Type is set to New Items are added. Leave all other setting in the defaul postion.
  4. Click Ok at the bottom of the list.
